Liquid Personnel is recruiting Social Worker for its client's Adults Discharge to Assess Team at Bristol. This is a fantastic opportunity to support citizens during a critical stage in their care journey, ensuring safe and effective transitions from hospital to community settings.
What will your responsibilities be?
Assess and support citizens on discharge from hospital across three discharge pathways
Complete post-discharge assessments and reviews in line with the Care Act
Undertake Care Act Assessments (CAA) and support planning
Carry out Best Interest (BI) decisions and work within the Mental Capacity Act (MCA)
Manage safeguarding concerns in line with statutory responsibilities
Provide duty cover where required
Attend Quality Assurance (QA) panels to support funding decisions

Qualifications & Experience:
To be successful in this role you must have,
Qualified Social Worker with relevant Social Work England registration
Experience within Adults Services, ideally discharge or hospital-based teams
Strong knowledge of Care Act, MCA, and safeguarding legislation
Ability to work in a fast-paced environment and manage competing priorities
